The information I provide on this website is not intended to replace consultation, evaluation, services or establishment of a licensed practitioner-patient relationship. It is not diagnostic and does not replace an in-person examination and agreed upon course of action. It is not being used, or to be implied, as a means of practicing outside of state or scope of licensure. The material is meant for information and possible supplementation of what is provided by your specific healthcare provider. It should be reviewed with that person, also.

CANCELLATION POLICY:

There is a strict 48 hour cancellation on all appointments. A full charge will be taken.

PRIOR TO INITIAL ASSESSMENT:

I will need you to fill out some forms prior to your first appointment. Please have them filled out before the appointment. If you decide to fill them out during the appointment an extra fee will be charged. If you have not filled every part of the form and leads to me speanding time asking more questions an extra fee will be charged. If you are unable to print your forms please let me know 48 hours before and I can arrange for the printing. This means you will have to come at least 30 mins before your appointment to fill these out in my reception area

FACILITIES:

Unfortunately I do not have disabled access to my clinic. Please note the clinic is used by other health professionals. Please be respectful of other patients and professionals whilst at the clinic. Please arrive 10 minutes early only. If you are earlier there are many coffee shops in the area for you to wait. Due to the size of the clinic and the large foot fall through the clinic we cannot allow patients to be more than 10 minute early. Please let the receptionist know you have arrived for an appointment with myself. I will come and collect you once I am ready. 

MEDICAL CONCERNS:

All medical concerns must be highlighted during the initial appointment. If you have any changes in your medical condition then please notify me ASAP.
